Requirements for Becoming a Roots Surrogate
Surrogacy is extraordinary work—and not everyone is the right fit. At Roots, we’re intentional about who joins our community because we want every surrogate to have the safest, healthiest, and most supported journey possible.
You are between 22-40 years old.
You do not use illegal drugs, smoke cigarettes, or abuse alcohol.
You are not currently using any medications for mental health issues.
You have delivered a child of your own and have at least 50% custody.
You have healthy, full-term pregnancy(ies) and deliveries without serious complications.
You have a healthy height/weight ratio (BMI under 32). Calculate here.
You do not participate in government aid programs such as cash assistance, welfare, public housing, or Section 8.
You have the support of family, friends, and your partner (should you have one).
You must be willing to follow doctor's requirements for testing, medications, and vaccine requirements. COVID vaccine is not required.

Becoming a surrogate is a meaningful journey, and you should never have to navigate it alone. Our team has guided hundreds of surrogates through this process, so we know what each stage looks like and how to prepare you for what comes next. Apply & Pre-Screen
Start with a short application and prescreen to confirm you meet the basic qualifications before moving forward in the screening process.
Onboarding
We will ask for some documents from you, run a background check and your medical records and get to know you better on a call. You will do a psychological evaluation.
Match with Intended Parents
We'll help you find Intended Parents (IPs) who align with your values and preferences, and then schedule a match meeting so you can get to know them.
Medical
You'll complete a thorough medical screening at the intended parents' fertility clinic. You may have to travel to do this, but you'll be paid for all costs.
Legal
Review and sign a legal contract with the IPs to outline everyone's rights, responsibilities, and expectations with a lawyer we provide.
Take Medication
Start taking your IVF medication.
Embryo Transfer
Begin a course of fertility medications and undergo the embryo transfer procedure. After 10 days, we'll see if you're pregnant!
Pregnancy
You'll have regular prenatal appointments and open communication with the IPs. Base comp payments are paid equally over the next 8 months of pregnancy.
Delivery
Deliver the baby and experience the joy of helping a family grow. The IPs get to hold their baby for the first time. Our favorite part of the process
Postpartum
Take time to rest and recover after delivery. We provide a lot of support throughout this period, the fourth trimester!

Reflect on your journey and celebrate the life-changing impact you've had, and join our Roots community of surrogates and IPs after the process.
